A randomized, double-blind study of tubal sterilization was performed to determine the effect of the operation on pregnancy rates and on subsequent fertility. Seventy-five patients underwent either hysterosalpingectomy (n = 43) or simple salpingectomy (n = 32) at the time of oophorectomy. Thirty-two patients (25 in the hysterosalpingectomy group and seven in the salpingectomy group) who had achieved at least one pregnancy within two years before tubal ligation were matched for parity, age, cycle length, duration of infertility, number of years since the last conception, and length of menstrual cycle. Follow-up was performed by chart review, telephone contact, and an interview. Forty-five (60%) of the 74 women who were interviewed responded to the question: "Did you have intercourse after tubal ligation?" Among the women who responded, the fertility rate was significantly lower in those who had undergone tubal ligation (p less than 0.01). Although seven of the seven women in the salpingectomy group and nine of the 25 women in the hysterosalpingectomy group became pregnant within five years of the sterilization, the difference between the two groups was not significant (p = 0.23). All of the seven of the seven patients in the salpingectomy group and 14 of the 25 patients in the hysterosalpingectomy group who had conceived became pregnant within a year of tubal sterilization. Ten of the 11 women who had undergone tubal ligation to achieve sterility conceived within two years of sterilization. Three patients conceived more than four years after tubal sterilization. The data suggest that tubal ligation reduces fertility, but the level of fertility loss is not consistent and permanent..
